Section 4 – Declaration
Whizz Kidz is committed to safeguarding and protecting all children, young people and adults at risk by implementing robust safer recruitment practices. Whizz Kidz conducts Disclosure and Barring Service (DBS) or Protecting Vulnerable Groups (PVG) checks on applicable positions as part of this process. A conviction will not necessarily prevent you from volunteering, but it will be taken into account when assessing your suitability. Failing to disclose a conviction which then appears on your criminal records check may exclude you from volunteering with Whizz Kidz. All cases are assessed as per the Whizz Kidz Criminal Record Disclosure Policy.

I also grant permission for Whizz Kidz to conduct any necessary background checks as part of the application process. Section 5 – Data Protection and Confidentiality Commitment
Volunteers supporting Whizz Kidz may work with and overhear confidential personal information relating to service users, beneficiaries, volunteers, donors, employees, partner organisations, suppliers and other third parties. 
 
All volunteers are required to maintain the confidentiality of personal information and must follow Whizz Kidz Privacy and Data Protection Policy and related policies and procedures.  
 
When volunteering with Whizz Kidz, you agree:
- To treat all information about people – e.g. service users, donors and staff – as confidential.
- To adhere to Whizz Kidz Privacy and Data Protection Policy and related policies as listed in the Privacy and Data Protection Policy.
- To only disclose personal information in accordance with the Whizz Kidz Privacy and Data Protection Policy.
- That any non-compliance with Whizz Kidz Privacy and Data Protection Policy will be treated as misconduct and may result in Whizz Kidz terminating your volunteer role.
- That all confidential information about Whizz Kidz – that is, information not available in the public domain – must not be disclosed.
Personal Information

To volunteer with Whizz Kidz, we need to keep some personal details, including special categories of personal data, about you on our database. We need to keep these records so that we can administer and manage Whizz Kidz efficiently, meeting our legal obligations as a volunteer manager and as a charitable organisation.

The personal information will be available to limited Whizz Kidz employees and volunteers with a need to access the information, to others authorised by you and/or by us to receive the information and to those to whom we are legally obliged to disclose it. By signing this Agreement you confirm your understanding of how Whizz Kidz will handle your information.

Section 6 – Communications Consent
From time to time, Whizz Kidz would love to stay in touch and update you on our work with young wheelchair users. Please place a cross in one or more of the boxes below if you would like to find out about Whizz Kidz’s services and activities, including updates on fundraising, events and other ways you can support us.

Section 7 – Photography and Videography Consent
We often take photographs and videos at our services, events and clinics. It’s a great way to show the work that Whizz Kidz does, to help us increase awareness and raise the vital funds we need to continue our work. By signing this form, you give your consent for Whizz Kidz to take photographs, videos and audio recordings of you and to use them in Whizz Kidz’s communications, marketing, awareness raising and fundraising campaigns.

Declaration:

I authorise Whizz Kidz to take/ and or use pictures, photographs, video recordings, audio recordings, digital images, and the like of me (the "Images") for any lawful purpose in connection with Whizz-Kidz's charitable objectives.

Whizz Kidz can include the Images of me in any of the following:

• Printed materials (posters, flyers, leaflets, booklets, reports);
• Online media (websites, social media);
• Broadcast media (television and radio);
• Advertisements;
• Presentations;
• Fundraising applications/mailing campaigns; and
• Any other use where the aim is to further Whizz Kidz's charitable objectives.

If the Images are accompanied by a description, Whizz Kidz can include my name in the description.

Furthermore,

• I assign to Whizz Kidz any interest in copyright or other intellectual property rights that I may have in the Images.
• I will not object to or restrict Whizz Kidz's use of the Images for the purposes specified above.
• I will not bring any claims for damages against Whizz Kidz in connection with their permitted use of the Images.
• I understand and agree that Whizz Kidz will not pay me, now or in the future, for any use of the Images.
